During a Live match, you can go to the Formation Tab and from your substitution players you can choose who you want to enter the match and drag it to the one you want him to substitute.

Also, like in real football, the substitutions will be made when the ball goes out.

However, if you do not attend a Live Match the game will automatically substitute players, starting with the best possible option from your team.
